This 4-day block repeats itself continuously in multiple 4-day blocks usually lasting 1–2 weeks before a period of leave for the hospitalist or rotating into another service. Patients will remain under the hospitalists’ care for i) the duration of their inpatient stay, ii) until discharge, or iii) are handed over to another hospitalist when their initial hospitalist has time off. The rest of the team remains constant, preserving the continuity of care.
